Route 138 is one of the few trans-Rhode Island routes. This one services the southern part of the state. The expressway section of Route 138 was originally to be extended westward to I-95 as part of the second plan for I-895, which would have provided freeway access from I-95 to Newport, Middletown, Portsmouth, and into Warren on the Mount Hope Bridge. From Warren, it would've followed the Route 136 corridor into Massachusetts to end at I-195. For information on the first plan for I-895, please see Route 37. Route 138 runs 45.2 miles in Rhode Island.

Exeter - 0.5 miles: Connecticut State Line to Hopkinton Line
Spring Street

Hopkinton - 5.2 miles: Exeter Line to Richmond Line
Spring Street, Nooseneck Hill Road and Main Street

Richmond - 6.0 miles: Hopkinton Line to South Kingstown Line
Main Street and Kingstown Road

South Kingstown - 8.9 miles: Richmond Line to North Kingstown Line
Usquepaugh Road, Kingstown Road, Moorsefield Road and Tower Hill Road

North Kingstown - 5.0 miles: South Kingstown Line to Jamestown Line
Tower Hill Road, Expressway and Jamestown-Verrazano Bridge

Jamestown - 3.2 miles: North Kingstown Line to Newport Line
Jamestown-Verrazano Bridge, Expressway and Claiborne Pell (Newport) Bridge

Newport - 1.6 miles: Jamestown Line to Middletown Line
Claiborne Pell (Newport) Bridge, Bridge Access Road and Admiral Kalbfus Road

Middletown - 3.2 miles: Newport Line to Portsmouth Line
West Main Road and East Main Road

Portsmouth - 8.5 miles: Middletown Line to Tiverton Line
East Main Road and Fall River Expressway

Tiverton - 3.1 miles: Portsmouth Line to Massachusetts State Line
Fall River Expressway and Main Road
